2. Chest Pain
You may feel chest pain when fluid builds up around your lung. This pain often feels sharp and gets worse when you take a deep breath. Coughing or moving can also make it hurt more. The fluid presses on the lining around your lungs and irritates it. When this lining rubs during breathing, you feel a stabbing or aching pain.
You might notice the pain on one side of your chest. It can stay in one spot or spread to your shoulder or upper back. Some people feel mild discomfort, while others feel stronger pain that limits deep breathing. Large amounts of fluid can increase pressure inside your chest. This pressure may cause a heavy or tight feeling and lead to short, shallow breaths.
Chest pain from pleural effusion can feel similar to pain from pneumonia, pleurisy, or even a blood clot in the lung. Many conditions can cause chest pain, so careful attention to other symptoms such as shortness of breath, cough, or fever can support proper diagnosis and care.
3. Cough
A cough is a common symptom when you have pleural effusion. Fluid builds up between your lung and chest wall. This extra fluid can irritate your lung and trigger a cough. The cough often starts as your body reacts to pressure and irritation in the chest.
You may notice that your cough is dry and does not bring up mucus. It can feel persistent and annoying, especially when you take a deep breath. As the fluid increases, your lung cannot expand fully. Your body reacts by trying to clear the irritation, which leads to more coughing. You might cough more when you lie down or change positions.
If an infection such as pneumonia caused the fluid buildup, your cough may produce mucus. Fever or chest discomfort may also occur. In cases linked to lung cancer or long-term lung disease, the cough may last longer and slowly worsen.
4. Hiccups
Hiccups can occur when fluid builds up around your lungs and irritates the diaphragm. The diaphragm is the main muscle that helps you breathe. When it becomes irritated, it can tighten without your control and cause the familiar “hic” sound. This reaction happens because the muscle starts to spasm.
You may notice short bursts of hiccups that last a few minutes. In some cases, they can last much longer and return often. Ongoing hiccups can feel uncomfortable and tiring. Pleural effusion can press on nearby tissues and disturb the nerves that control your diaphragm. When these nerves send mixed signals, your diaphragm may spasm again and again.
Hiccups linked to pleural effusion often appear with other symptoms. You might also have shortness of breath, chest pressure, or a dry cough, but most hiccups are harmless and brief. However, hiccups that last for days or keep coming back may signal irritation inside your chest, and medical evaluation may be necessary.
5. Rapid Breathing
You may notice that you are breathing faster than normal, which is called rapid breathing. It can happen when fluid builds up around your lungs and limits how much they can expand. The extra fluid reduces the space your lungs need to fill with air.
When fluid collects in the pleural space, your lungs cannot fully expand. Your body reacts by increasing your breathing rate to bring in more oxygen. You may take short, quick breaths without meaning to. This response is your body’s way of adjusting to the pressure around your lungs.
Rapid breathing often comes with a feeling that you cannot get enough air. You might feel the need to sit upright or lean forward to breathe more easily. Even light activity, such as walking across a room, can make your breathing speed up. In some cases, your breaths may also become shallow and less effective.
6. Fever
You may develop a fever if an infection causes your pleural effusion. A fever means your body temperature rises above normal, often over 100.4°F (38°C). It shows that your immune system is fighting a problem. In this condition, fever often links to lung infections such as pneumonia.
The infection can irritate the lining around your lungs and cause fluid to build up. If the fluid becomes infected, it can turn into pus, which makes fever more likely. You might also have chills or shaking, night sweats, tiredness, weakness, or loss of appetite.
Fever does not occur in every case. If heart failure, kidney disease, or low blood protein levels cause the fluid buildup, your temperature may stay normal. When infection is present, antibiotics are often used to treat bacteria. If a large amount of infected fluid collects, drainage may be required to control the infection and reduce fever.
7. Chills
Chills can happen when your body tries to fight an infection linked to pleural effusion. You may feel sudden cold sensations, even in a warm room. Your teeth might chatter, and your body may shake without control. These chills often appear along with a rising body temperature.
Chills often come with a fever. When fluid builds up around your lungs due to infection, such as pneumonia, your immune system reacts. This response raises your body temperature and triggers shivering to create heat. You may also notice sweating, headache, body aches, chest pain when breathing, cough, rapid breathing, or fatigue.
Not all pleural effusions cause chills. They are more common when infection is the cause rather than heart failure or other non-infectious problems. Strong or lasting chills may point to an active chest infection. Imaging tests and fluid samples are often used to check for bacteria, and antibiotics may be prescribed if needed.
8. Night Sweats
You may notice heavy sweating during sleep if you have pleural effusion. The sweat can soak your clothes or sheets. This symptom often connects to the condition that caused the fluid buildup, not just the fluid itself. Night sweats can be one sign that your body is under stress.
Infections such as pneumonia or tuberculosis can trigger night sweats. These infections can inflame the lining around your lungs and lead to extra fluid in the chest. Cancer that affects the lungs or pleura can also cause night sweats. Fever, unplanned weight loss, ongoing cough, chest pain, and shortness of breath may appear at the same time.
When your body fights infection or deals with cancer, it may raise your internal temperature. Your body then sweats to cool down, even if your room feels cool. Night sweats alone do not confirm pleural effusion. However, when they occur with breathing problems or chest discomfort, further medical evaluation may be needed.
9. Difficulty Breathing When Lying Down
You may notice that your breathing becomes harder when you lie flat on your back, which is called orthopnea. It often improves when you sit up or prop yourself up with pillows. The change in position affects how fluid rests around your lungs.
Pleural effusion causes fluid to build up in the space around your lungs. When you lie down, this fluid can spread out and press more evenly against your lungs. This pressure limits how much your lungs can expand, which makes breathing feel shallow or tight. The discomfort may become more noticeable at night.
You might need two or three pillows to sleep comfortably. Some people sleep in a chair because breathing feels easier in an upright position. Orthopnea can also occur with heart or lung conditions. With pleural effusion, the fluid outside the lung restricts movement.
10. Unexplained Weight Loss or Loss of Appetite
You may notice that you are eating less than usual without trying. Food may not seem appealing, or you may feel full after only a few bites. Over time, this can lead to weight loss that you did not plan. These changes can happen slowly and may not seem serious at first.
Pleural effusion can make you feel tired and short of breath. When breathing feels difficult, you may not have the energy or desire to eat. Chest discomfort can also lower your appetite. Eating may feel like more effort when your body is already working hard to breathe.
Sometimes the condition causing the fluid leads to weight loss. Heart failure, lung infections, or cancer can reduce appetite and cause gradual weight loss. Doctors define unintentional weight loss as losing about 5 percent of your body weight within 6 to 12 months without trying. This level of weight loss may need medical attention.
