2. Folic Acid Deficiency

Folic acid, also called vitamin B9, helps the body make new cells. It supports red blood cell production and helps tissues grow and repair. Leafy green vegetables, fruits, beans, and fortified grains are common sources. Because the body does not store large amounts of folic acid, a steady intake of food or supplements is needed.

When intake stays low for weeks or months, deficiency can develop. Folic acid deficiency often occurs in people with alcohol use disorder, eating disorders, long-term poor diet, or gut problems that limit nutrient absorption. These conditions can lead to several nutrient deficiencies at the same time.

A lack of folic acid does not directly cause hypokalemia. However, the same poor intake or absorption problems can also reduce potassium intake and gradually lower potassium levels. Symptoms of folic acid deficiency include fatigue, weakness, and pale skin because the body produces large, abnormal red blood cells. Potassium levels may fall further when other factors such as vomiting or diarrhea increase potassium loss.

3. Prolonged Vomiting

Continued fluid loss can also reduce blood volume. In response, the body releases hormones that help retain sodium and water. Vomiting also causes loss of stomach acid, which raises the blood’s pH level. As the blood becomes more alkaline, potassium shifts from the blood into the cells, causing the measured potassium level to fall further. People with ongoing vomiting may develop muscle weakness, cramps, constipation, or irregular heartbeats.

The risk becomes higher when vomiting continues for several days or occurs with poor food intake. Low magnesium levels may also develop during prolonged vomiting, which can make potassium replacement more difficult. Severe cases may also cause dizziness or low blood pressure because ongoing fluid loss reduces circulation.

4. Diabetic Ketoacidosis

Diabetic ketoacidosis is a serious problem that can happen in people with diabetes, most often type 1. It develops when the body does not have enough insulin, so cells cannot use glucose for energy. The body then starts to break down fat for fuel at a rapid rate, producing acids called ketones.

As ketones build up in the blood, the blood becomes too acidic. At the same time, high blood sugar causes the kidneys to pass more glucose into the urine. Water and electrolytes leave the body with it, leading to dehydration and gradual potassium loss.

Even if blood tests first show normal or high potassium, total body potassium is often already low because much of it has been lost in urine. As potassium levels fall, a person may develop muscle weakness, leg cramps, fatigue, or constipation. In more severe cases, low potassium can affect heart rhythm and cause abnormal beats. Ongoing diarrhea needs medical attention, especially if weakness, irregular heartbeat, or signs of dehydration develop.

7. Excessive Alcohol Consumption
