This may happen after procedures meant to fix ptosis or remove excess skin, and it can also occur after other eye surgeries, such as cataract removal. During ptosis repair, a surgeon tightens or reattaches the muscle that lifts your lid.

If the muscle heals in a weak position, your eyelid may sit too low. In some cases, the lid may sit too high or look uneven. Swelling after surgery can also make your eyelid look droopy. This swelling usually improves as you heal. If the tissue does not heal as planned, the droop can last longer.

Scarring under the skin may change how your eyelid folds. You might notice that one lid looks different from the other. Uneven healing can affect the shape and height of your eyelid. Dry eye problems can worsen after surgery because when your eye stays more open, the surface can dry out, and this irritation may make your eyelid feel heavy or tired.

Botulinum toxin injections can cause a droopy eyelid if the drug spreads beyond the target muscle. This spread can weaken the muscle that lifts your upper lid. When that muscle weakens, your eyelid may sag and partly cover your eye. This side effect is temporary in most cases.

Reports show it can happen in about 1 out of 20 injections around the eye. Your eyelid often returns to normal within about two weeks as the drug effect fades. Serious problems are rare, but in very uncommon cases, the needle can injure the eye. Improper technique or counterfeit products increase the risk of harmful reactions. If your eyelid droops after treatment, the effect usually wears off within about three months as the medication loses strength.

This pressure may cause gradual drooping that worsens over time.

5. Congenital Ptosis

Congenital ptosis means you are born with a droopy upper eyelid. You or your child may notice it at birth or within the first year of life, and it can affect one eye or both eyes. This condition usually happens because the muscle that lifts the eyelid did not develop the right way before birth.

When that muscle is weak, it cannot raise the eyelid fully. As a result, the lid may cover part of the eye. In mild cases, the drooping may only change how the eye looks. In more severe cases, the eyelid can block part of your vision.

If the lid covers the center of the pupil, it can interfere with normal vision development in children. When vision is blocked for a long time, the weaker eye may not develop clear sight, which can lead to lazy eyes, also called amblyopia. Some children with congenital ptosis may tilt their head back or raise their eyebrows to see better. These habits can strain the neck or forehead muscles over time.

Sometimes your droopy eyelid has no clear cause, which is called idiopathic ptosis. This means tests and exams do not show a nerve problem, muscle disease, injury, or growth. You may notice the lid droops slowly over time, and it can affect one or both eyes. In many cases, the muscle that lifts your eyelid weakens without a clear reason.

Your eyelid may stay the same for years or slowly worsen over time. The droop can be mild and only change how you look, or it can block part of your vision.
